This requirement will be used for Annual Hazardous Materials CUPA Permits for the VA Loma Linda Healthcare System (VALLHS), located at 11201 Benton Street, Loma Linda, CA. 92357. The Safety Service located at VA Loma Linda HCS requires an Annual Hazardous Materials Certified Unified Program Agency CUPA Permit located at the medical facility. The CUPA permit is required for all facilities that handle hazardous materials or generate hazardous waste. The unified program consolidates the administration, permit, inspection, and enforcement activities. The CUPA program is designed to consolidate, coordinate, uniformly and consistently administer permits, inspection activities, and enforcement activities throughout San Bernardino county.
