X--Northport VA Day Care Lease

U.S. DEPARTMENT OF VETERANS AFFAIRS DESIRES TO OUTLEASE SPACE The Department of Veterans Affairs, is seeking to outlease space specific to the Northport VA Healthcare System for the purpose of establishing and operating a Day Care / Child Care Center located at the VA Northport Campus, the site location Northport Employee Day Care 79 Middleville, Bldg 21 Northport, NY 11768. The space to be outleased in Bldg 21 consists of 8,444 sq. ft., to be used exclusively as a not for profit day care center. The Government will provide security, building maintenance, alarm system, fire protection services, snow removal, pest control services, grounds maintenance, emergency medical services as necessary, parking for bona fide employees of the day care center to the extent that space is available, and utilities to allow operation of the day care / child care center. Utilities are defined as electricity, water, gas, and sewage. Potential Day / Child Care provides seeking to lease the space with Department of Veteran Affairs regarding the above referenced location are required to meet both VHA and New York State child care regulations and standards. Additionally, potential child care providers will be required to submit past performance and qualifications and include the following certifications and licensure documentation: " Copy of New York Office of Children and Family Services Day Care Center Licensure " Copy of Internal Revenue Service tax exempt form-if applicable " Certificate of Article of Incorporation or LLC documentation depending upon business type. " Copy of Child Care Center's List of Board Members or Executive Committee to include the member(s) authorized to sign the lease " Copy of mandatory insurance certification for fire damage " Copy of accreditation from the National Association for the Education of Young Children, if you do not have this accreditation, if a lease award is made, one will be required after one year of operation. A standard lease for three (3) years will be considered. Furthermore, the potential day / child provider who due to occupy the available space shall be required to provide a reduced rate of 25% per week for VA employees and affiliates thereof.
