This is a sources sought request for commercial items. The Defense Centers of Excellence Hyperbaric Oxygenation TBI Program is seeking a business concern with current relevant qualifications under NAICS code 312112 and is a current GSA schedule holder to provide bottled water services to participating sites. 01. STATEMENT / SCOPE OF WORK The Defense Centers of Excellence Hyperbaric Oxygenation TBI Program requires that the services provided shall meet or exceed the following: Case packed bottled water, 0.5L bottles quantity of 24 or 5 gallon bottles used in conjunction with a water cooler and cup dispenser. If the Contractor will deliver 5 gallon bottles they will be responsible for providing cups, collecting emptied 5 gallon bottles and retrieving the water cooler after the period of performance. Contractor shall provide all labor, supervision, materials, and equipment necessary to fulfill the requirement. The locations which services are needed are: Fort Gordon, Augusta, Georgia; Camp Lejeune, North Carolina; Fort Carson, Colorado; and the Outcomes Assessment Center at Colorado Springs, Colorado. Camp Pendleton, California and Joint Base Lewis, McChord Washington, sites will be added at a future date. Each bottle of water shall be marked with the manufacturer's original label, and each bottle opening shall have the original manufacturers tamper proof seal or cap intact. Shipping must be free on board (FOB) destination CONUS (Continental U.S.), at the specified destinations with on-site inspection and acceptance. Approximate office size is 25-30 persons at each site. Regional sources are sought for twice a month delivery schedules. Site access will need to be coordinated prior to the first delivery with the project officer and the Contracting Officer's Representative. The anticipated period of performance will be two year or three years to single or multiple contractors. 02. DETAILED DESCRIPTION OF SERVICE / PRODUCT NEEDED: If the Contractor will be providing 5 gallon bottle service, then the following items are required: 120 volt portable hot/cold electric water coolers, bottled drinking water suitable for use in the portable electric coolers with manufacturer's original seal, paper cups and paper cup dispenser. Emptied 5 gallon bottles are to be retrieved after full bottle delivery. At the end of the period of performance or in the event of premature termination of site participation the Contractor will retrieve provided equipments. If the Contractor will be providing case packed bottled water the following items are required: 24 bottles per case, each bottle must contain a minimum of 0.5L of non-carbonated water; manufacturer's original seals must be intact. 03. NEEDS FOR RESPONSE: Respondents must provide for evaluation GSA schedule for pricing, proposed delivery schedule, proposed part numbers for equipment furnished, procedures for returning furnished equipment and delivery areas covered.
