Q -- Dental Assistants

The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items to provide 4 Dental Assistant required for Government beneficiaries. The contractor shall provide care at the 366th Medical Group, 90 Hope Drive, Building 6000 also referredt o as the “medical treatment facility” (MTF) herein. Contractor care shall cover the range of services provided in a civilia medical treatment faciity performance shall be according to the requirements contained in this performance work statement (PWS), professional standards of the Joint Commision on Accreditation of Healthcare Organization (JCAHO), and to AFI 47-101 as well as appropriate dental operating instructions, which will be provided to the Contractor. The following data is provided for information only. Present department staff consists of: 6 providers, 17 dental assistant technicians, 3 dental laboratory technicians. Dental visits for the last 12 months: 15,810. The solicitation will result in a firm fixed-priced contract with a base year and four one-year options. Base Year Period of Performance will be from 1 Oct 05—30 Sept 06. The Standard Industrial Classification (SIC) for this acquisition is 8049 and the North American Industry Classification System (NAICS) code is 621399. This action is being advertised as a 100% small business set-aside. A formal Request For Proposals (RFP) will be issued on or about 6 June 2005 via Fed Biz Opps.
