The solicitation number is W81K00-14-T-0186 and is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74 and Defense Federal Acquisition Regulation Supplement Publication Notice (DPN) 20140528. This solicitation contains items for delivery to both San Antonio Military Medical Center (SAMMC), Joint Base San Antonio - Fort Sam Houston, Texas AND Reynolds Army Community Hospital (RACH), Fort Sill, Oklahoma. CLINs 0001 - 0018 apply to SAMMC and CLINs 1001 - 1010 apply to RACH. One purchase order will be issued for each delivery location, with each purchase order to be issued on an all-or-none basis.
