AMENDED The VA Pittsburgh Healthcare System intends to negotiate on a sole source basis with Klingensmith Inc. 404 Ford St, Ford City, PA 16226-1230 for home oxygen services. Contractor will provide services to approximately 960 patients per month. The need includes providing in home supplies such as the following: CPAP/BIPAP machines and replacement parts; Concentrator (Rental) with two broad categories' of flow rates and replacement parts; various cylinder sizes (rental) to provide oxygen [Ex. A (smallest)-E (largest)] and replacement parts; Oxysafe Firesafe Cannula; Procurement/Distribution of two types of oxygen and backup/spare parts; Nasal Cannula/Masks and replacement parts; Portable oxygen systems (used when a Veteran travels); Delivery of VA owned equipment to include setup (CPAP/BiPAP/Portable Oxygen Concentrator (POC)/Oxysafe Firesafe Cannula). The services associated with this contract include: All logistical/administrative functions involved with the day to day operation of providing the service to the Veteran and their families, including in home respiratory therapy visits. This includes providing a Quality Control Plan (QC) plan that is overseen by the Prosthetics Quality Assurance Evaluators (QAE), coordinating and managing patient care, deliveries consisting of pickup and delivery of supplies, briefing patient on proper use of equipment, documenting use of supplies and inspection of home in accordance with The Joint Commission (TJC) standards concerning patient use/safety of home oxygen. Requiring activity is not seeking requests for competitive quotes; however, any firm that believes that it can provide the requirement above may give written notification to the Contracting Office.
