The Department of Veterans Affairs (VA) has an ongoing program to furnish artificial limbs, prosthetics, orthotics, and related services to those veterans that require them and are eligible for assistance. It is VA’s intention to solicit proposals and award multiple contracts on a competitive basis to providers within the following VA Sierra Pacific Network, Veterans Integrated Service Network (VISN) 21 jurisdiction: VA Central California Health Care System (HCS), Fresno, VA Northern California HCS, Sacramento, VA Pacific Islands HCS, Honolulu, VA Palo Alto HCS, Palo Alto, VA Medical Center (VAMC), San Francisco, and VA Sierra Nevada HCS, Reno. At least one contract within each VISN 21 medical center/health system geographical area will be set aside for small business concerns. Contractors shall fabricate all artifical limbs and orthotic appliances in strict conformance to the prosthetic prescriptions provided by the VA Amputee Clinic Team. Contractors shall not add or substitute components or alter the beneficiary’s prescription in any way without prior approval and written authority from the VA Amputee Clinic Team Prosthetic Representative or designee. Period of performance is October 1, 2005 or date of award through September 30, 2006 with two (2) one-year option periods, subject to availability of funds.
