The applicable North American Industry Classification System (NAICS) is 621111. The Bureau of Reclamation, Pacific Northwest Regional Office, provides acquisition/administrative support services for the Dept of Labor's Columbia Basin Job Corps Center in Moses Lake, Washington. The Center has a requirement for professional medical services for approximately 250 students enrolled at the Center. Services are to be performed at the Center. Services include, but are not limited to: initial routine physical exam of enrollees, medical history, diagnosis and treatment of illness and/or injury, immunizations, x-rays, referrals to specialists, on-call services, 24-hour emergency coverage, and access to inpatient care. Physicians and health care providers performing services must be licensed in the state of Washington. The health care providers will be required to provide 16 hours of service per week minimum at the Government-furnished office/clinic at the Center. The contractor must also have available, or make arrangements to have available, facilities within the local commuting area of Moses Lake, WA for 24 hour on-call emergency services. Reclamation intends to award one fixed-price contract. The period of performance shall be for one year (base year) beginning approximately July 1, 2005, through June 30, 2006, with four option years to renew. These services will be purchased in accordance with FAR Part 15. Any award will be based on the best value to the Government. This requirement is SET-ASIDE FOR SMALL BUSINESS. We will evaluate past performance, experience, and price and price related factors to determine the best value. These factors are subject to change and the offerors will need to review sections L and M of the solicitation when the solicitation is issued.
