The VA Pittsburgh Healthcare System (VAPHS) intends to negotiate on a Sole Source basis with Qiagen, Inc., 27220 Turnberry Lane, Suite 200, Valencia, CA 91355, to award a Base and Three Option Year Contract (estimated total to be $44,032), to provide annual Maintenance Services for VAPHS Qiagen, Inc. Qiagility; Qiacube; and RGQ MDX Equipment, in accordance with the following Scope of Work and Security Requirements: Statement of Work 1. Title: Service of Qiagility, Qiacube, and the RGQ MDx 2. Scope of Work: The service contract will include onsite repair within 2 business days of the call; yearly, onsite preventive maintenance; cost coverage for repair parts, labor, and travel. Security Requirements: Contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police. VA ID badge must be worn at all times while on VA premises. Contractor owned computer equipment including laptops are not permitted to be connected to the VA network. Any removable storage device used in medical equipment must be scanned by Biomed or OIT. The following language from VA Handbook 6500.6 is required in this contract: o Appendix C: Paragraphs #3a, #5h, #6a, #7a, #7d, #9(all) o Appendix D - Before being granted access to VA information or information systems, all contractor employees and subcontractor employees requiring such access must sign the VA's Contractor Rules of Behavior. Contractor must complete "VA Privacy and Information Security Awareness and Rules of Behavior" training prior to the performance of the contract and annually thereafter. Training must be completed in VA's TMS system (https://www.tms.va.gov/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The COR must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the COR.
