The VA Pittsburgh Healthcare System intends to negotiate on a Sole Source basis with Qiagen, Inc., 27220 Turnberry Lane, Suite 200, Valencia, CA 91355, to provide a one-time Preventative Maintenance Service on the Department of Veterans Affairs Qiagen Pyro Q24 machine, in accordance with the following Scope of Work and Security Requirements: Statement of Work (SOW) A. GENERAL GUIDANCE 1. Title: Qiagen PyroMark Preventative Maintenance 2. Scope of Work: The contractor shall provide all labor, parts, and all other resources required to deliver the requirements stated herein. 3. Background: VA Pittsburgh has the following equipment: (1) Qiagen PyroMark, serial number 9001671_001616-14B and VHAPTH equipment ID number 351129, which needs regular preventative maintenance (PM). 4. Service: One-time Preventative Maintenance Service. 5. Type of Contract: firm fixed price. C. GENERAL REQUIREMENTS 1. Contractor shall notify Biomedical Engineering and the Police service when they arrive on site. 2. Contractor will schedule PM with Biomedical Engineering or the service line. D. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the Period of Performance of the Contract. Task One: The Contractor shall provide all parts and labor for preventative maintenance according to the Technical manual specific in Section A.3 of this SOW. Maintenance will be scheduled with VAPHS staff before coming on site and will be performed by staff trained by Qiagen on these devices. Deliverable One: A record, either hard copy or electronic copy, detailing the work performed in task one. E. SCHEDULE FOR DELIVERABLES Draft copies of deliverables shall be provided to the Biomedical Engineering staff at VAPHS before the contractor leaves the facility at each visit. If a final copy can be provided at that time, then a draft copy is not necessary. Final copies must be provided within 5 business days of the visit. G. REPORTING REQUIREMENTS Contractor shall provide documentation according to section D of this SOW. H. TRAVEL [If applicable] Devices are located at the University Drive Campus of VAPHS, all travel expenses are the responsibility of the contractor. Security Requirements: Contractor will be required to comply with physical security guidelines by either checking in with the VA Police each time they come on-site to perform contracted services or by obtaining a VA Contractor ID badge from the VA Police. VA ID badge must be worn at all times while on VA premises. Contractor owned computer equipment including laptops are not permitted to be connected to the VA network. The following language from VA Handbook 6500.6 is required in this contract: o Appendix C: Paragraphs #3a, #5h, #6a, #7a, #7d, #9(all) o Appendix D - Before being granted access to VA information or information systems, all contractor employees and subcontractor employees requiring such access must sign the VA's Contractor Rules of Behavior. Contractor must complete "VA Privacy and Information Security Awareness and Rules of Behavior" training prior to the performance of the contract and annually thereafter. Training must be completed in VA's TMS system (https://www.tms.va.gov/). Contractors must use the TMS Managed Self Enrollment method to complete the training in TMS. The COR must ensure that all contractors are validated in the PIH domain. Proof of training completion must be verified and tracked by the COR.
