STATEMENT OF WORK (SOW) 1. Contract Title Preventive and corrective maintenance services of nuclear medicine equipment. 3. Background The Medical Equipment listed in the section below is required to maintain clinical services currently provided at the Wilmington, DE VA Medical Center (VAMC). Failure of timely maintenance would force the facility to outsource these services, and inconvenience customers to a significant degree. 4. Scope This contract covers the equipment listed in the table below. Specific Tasks All coverage listed herein will be available M-F 8:00AM 5:00PM EST, excluding holidays. Contractor will guarantee 60 min initial response time for requests submitted via phone. Contractor will guarantee 4 hours response time for request that require on-site support. All parts will be delivered 10:30AM next day. Priority scheduling for service calls and access to spare parts inventory. The coverage includes equipment planned maintenance services per OEM manufacturer specifications performed during standard hours of coverage and unlimited technical services (24X7) and clinical applications telephone support (M-F, 8am-5pm). This service will include all parts, (excluding consumables) that fails during normal use, and service on all imaging equipment systems and subsystems, including but not limited to collimator changer, Workplace A/AP, general spare parts coverage, planned and unplanned services, phone support, all UPSs and integrated electronics cabinets, CT detectors, all X-Ray tubes, all remote services available, service history, and performance reports for all work performed. The contract also includes proprietary operating system software enhancements without hardware changes (excluding software upgrades). The contractor shall provide to the Wilmington VAMC written reports documenting all work performed within 5 business days of the completion of said work. All reports must be submitted via email to [email protected]. 6. Performance Monitoring The Technical POC shall monitor performance, to include submission of required reports. 7. Security The contractor shall have an existing MOU and remote access agreement in place. 8. Government-Furnished Equipment (GFE)/Government-Furnished Information (GFI). The VAMC will not provide service manuals or service diagnostic software to the contractor. The Contractor shall obtain, have on file, and make available to its FSE all operational and technical documentation (such as: operational and service manuals, schematics, and parts list), which are necessary to meet the performance requirements of this contract. 11.
