NOTICE TO INTENT TO SOLE SOURCE: The Department of Veterans Affairs Network Contracting Office 4, intends to award a Limited Source, Firm-Fixed-Price Contract to Chuddy Group LLC for the provision of providing trained Client Support Engineers and Field Service Technicians as needed to maintain an operational system for the ATP Packaging Unit and the Lower Packaging Unit owned by the Government. The service coverage shall provide 24X7 telephone support; on-site assistance Monday thru Friday 8am to 5pm to include all labor, travel, and parts and preventative maintenance shall be performed twice a year. Services to be located at the Wilkes-Barre VA Medical Center (WBVAMC), located at 1111 E End Blvd; Wilkes-Barre, PA 18711. This contract action is for services which the Government intends to solicit from only one source under the authority of FAR 8.405-6, Justification for Limited Source Award. Interested parties may identify their interest and capability to respond to the requirement or submit proposals. The North American Industry Classification System (NAICS) for this requirement is 519190 with a small business size standard of $27.5 million. THIS NOTICE OF INTENT IS NOT A REQUEST FOR COMPETITIVE QUOTATIONS. Responsible sources may identify their interest by submitting capability information by 12 Noon EST, on October 22, 2021, in order to establish the ability of the source to meet this requirement. A determination by the Government not to compete this proposal contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The Government will not be responsible for any cost associated with the preparation of responses to this notification. STATEMENT OF WORK Wilkes-Barre VA Medical Center INTENT: To provide telephone support, on-site support, and preventive maintenance service for the ATP Packaging Unit System and Lower Packaging Unit at the Wilkes-Barre VA Medical Center. SCOPE OF WORK: Contractor shall be a trained Client Support Engineer and provide all resources necessary to perform the following service: Contractor shall provide trained Client Support Engineers and Field Service Technicians as needed to maintain an operational system for the ATP packaging Unit and the lower Packaging Unit. Service coverage shall provide 24 x 7 Telephone Support, On Site Support Monday thru Friday 8am to 5pm to include all labor, travel, and parts. Preventive Maintenance shall be performed twice a year. Unlimited On-site service requests. PERIOD OF PERFORMANCE: The contractor shall complete the work required under this Statement of Work (SOW) for a period from 10/31/2021to 10/30/2022, unless otherwise directed by the Contracting Officer (CO) LOCATION OF WORK TO BE PROVIDED: Wilkes Barre VAMC 1111 East End Blvd. Wilkes Barre, PA 18711 GENERAL REQUIREMENTS: Contractor shall not leave any tools or equipment unattended. CONTRACTOR PERSONNEL SECURITY REQUIREMENTS: All contract employees shall have an identification badge on them at all times with their name and company clearly identified. Photo identification is not required; however, it is preferred. DELIVERABLES: Contractor shall report to the Pharmacy Representative to provide a report of the work performed. CHANGES TO STATEMENT OF WORK: Any changes to this SOW shall be authorized and approved only through written modification issued by the CO.
