T -- T-Printing

Single award contract for the period June 1, 2004 to May 31, 2005, for the Social Security Administration. This program will cover the conversion of electronic forms designed in FormFlow 2.22 into an Adobe Form Designer 5.0 format. Anticipate approximately 75-150 forms to be ordered. Each form will range from approximately 1 to 30 pages and the majority of the forms will consist of approximately four (4) unique pages. Each form will have an average of three (3) fillable forms. The hard copy form size will range from 5-1/2 x 8 inches up to and including 8-1/2 x 11 inches. These forms must be electronically designed to always print on 8-1/2 x 11 inch paper.
