The equipment must integrate with the existing Censitrac system already in place at the Bedford VAMC. The system must be authorized by the existing Censitrac system for mass upload instrument data to the live Censitrac application Because Censitrac Technologies is the only vendor/contractor that can provide the Censis Laser Marking Equipment, Installation, training, and live support for the life of the Censis Technology, this acquisition will not be competed. Objective The purpose of this Statement of Work (SOW) is to outline the tasks and responsibilities of the awarded Contractor in relation to the provision, installation, integration, upgrade, and implementation of the Laser Machine and provide education to the staff. 4. Scope The Scope of Work for the implementation of a Laser marking laser marking unit includes all planning, execution, and training for the client. Client will incorporate SOW into final contract. Upon receiving the final purchase order, Contractor will assign a project manager to act as the single point of contact for the customer in all matters involving the implementation. The project management functions follow the framework of the company s process and methodology for conducting implementations. Contractor will ensure it has adequate resources for accomplishing all phases of the project, including the training of the client s personal 5. Specific Tasks As part of the project, Contractor will be responsible for performing tasks throughout various stages of this project. The following is an overview of these tasks which will result in the successful completion of this project: Planning Phase: Lead a kick-off meeting with key client individuals, such as the client s project manager/leader, sterile processing department manager, Laser Safety Officer, and a representative from the hospital s IT department. The assigned project manager will provide an overview of the implementation methodology Work with the client to gather and refine requirements and scope of the project Create and present a detailed project plan including a schedule Execution Phase: Track the ordering of hardware Ship hardware Configure computer for laser unit control Install laser unit and workstation computer in accordance with all facility safety requirements Provide training on operation of the laser unit, marking and commissioning of instruments into Censitrac® System must be approved for use with the existing CensiiTrac software Project Handoff/Closure: Present a project closure report (PowerPoint) to client for review and approval Present a project training checklist showing that all training tasks have been completed. Procure and Install Hardware Ensure facility / department prepared for laser installation and setup Order and install power cabling (if required) Onsite Implementation and Training Provide staff for up to 32 hours of training during agreed upon time peri 6. Training Training will be provided by Contractor at the VA Bedford Facility and will last four days Contractor will ensure it has adequate resources for accomplishing all phases of the project, including the training of the Bedford VA staff/personnel. 7. Installation and Warranty: Contractor s Responsibilities: Based on approved deployment work schedule, the Contractor shall provide all labor necessary to install, deploy and configure the requirements of the Bedford VAMC. Contractor shall receive and place equipment in the pre-arranged designated areas of SPS (Bldg 10, Room 101) at the Bedford VAMC. The Contractor shall unpack and install all equipment in the designated area and dispose of all trash. Disposing of the trash is the responsibility of the Contractor. Contractor shall perform all installation and configuration necessary to complete the work, plus perform technical service checks to ensure product is fully operational in accordance with manufacture operating standards Vendor will coordinate all site visits with the Bedford SPS staff at least 24 hours in advance. The vendor will provide at-least a 1-year manufacturer warranty on the device from the date of installation.
