TRI-REGIONAL Modular RAMP INDUSTRY DAY ANNOUNCEMENT Service Area Office Central (SAOC) is sponsoring a Pre-Solicitation Conference Industry Days (IDS) on Wednesday July 30, 2014, to be held at the Minneapolis VA Health Care System (MVAHCS), One Veterans Drive, Minneapolis, MN 55415. There is no fee for this event. This announcement is not a contract, request for proposal, a promise to contract, or a commitment of any kind on the part of the Government. SAOC will not assume liability for costs incurred by an attendee or for travel expenses or marketing efforts; therefore, attendee cost in response to this notice is not considered an allowable direct charge to the Government. The Tri-Regional Modular Ramp IDS event is being conducted pursuant to Federal Acquisition Regulation (FAR) 15.201(e) "Exchanges with Industry before receipt of proposals" for the purpose of affording the government the opportunity to physically observe the various types of manufactured modular ramp products currently offered in the commercial market, and to obtain input from industry representatives with expertise and knowledge for the implementation and management of a full service modular ramp contract (assessment, material, and installation) . In addition, the Industry Day Conference is to provide preliminary information on SAO Central Region's anticipated technical requirements, business/contract considerations, acquisition approach as well as allowing each contractor the opportunity to express their technical, contractual and procedural concerns in regards to the Tri-Regional Ramp program. The Industry Day Conference will also provide an opportunity for contractors to ask questions and for interested firms to network with each other. The IDS will be held in two stages: A morning vendor modular ramp display and an afternoon dialogue session. Interested parties may register for either event or both. 1) Modular Ramp Display Presentation: During the morning of Wednesday, July 30, 2014 between the hours of 8:00 AM and 11:00 AM CT. Interested parties will be required to set-up their displays the evening prior to the event (07/29/14). 2) Government/Industry Dialogue Session: Will be held on July 30, 2014 between the hours of 1:00 PM and 4:00 PM CT at the MVAHCS Auditorium. The purpose of the conference is to exchange program ideas between industry and government. Display, dialogue session, and check-in procedure details/instructions will be provided upon receipt of the registration package. Industry Day participation is limited to three (3) representatives per company. Interested parties are required to pre-register for the event(s) of their choice. Attendees must present a current photo ID (driver's license, military ID, company ID or passport) matching the attendee information submitted via the Registration Form to gain entry into the event. Attendees are encouraged to arrive early to permit sufficient time for check-in. Written questions and comments will be accepted from all interested parties. [email protected] no later than 4:30 pm CT August 8, 2014. REQUIRMENTS The VHA provides ramps for veterans' homes as a part of the Prosthetics Clinical Management Program. The products and services required involve the installation and delivery of threshold, modular and portable ramp systems to the Veteran beneficiary's place of residence to accommodate access/egress issues at Veterans residences. Under this contract the contract awardee shall provide the product and services in accordance with a "turnkey" business concept as specified in this agreement in order to work effectively with VHA to meet the stated objectives. All ramp systems shall be of lightweight construction, durable, with a non-slip, skid-resistant surface, rust free, easily installed and removed, compact and recyclable. All ramp systems shall be installed in accordance with all applicable Federal, State, Local codes and VHA authorization. These supplies and services shall be provided when ordered by an authorized Contracting Officer or designated VHA personnel. 1.0 Areas of Responsibility and Jurisdiction: The Contractor shall provide services within the defined awarded area of responsibility/jurisdiction to the Veteran's place of residence, or other locations as indicated on the order from the Prosthetic Representative. 2.0 VHA Participants: All VA Medical Facilities within the SAO East, West and Central regions 3.0 Ramp Specifications: All ramp installations shall adhere to VA guidelines. While it is understood that ADA compliance is not applicable to residential properties, it is expected that submitted ramp designs will comply with ADA guidelines. If property restrictions prohibit strict adherence to ADA ramp guidelines, communication with the Contracting Officer or Prosthetic Representative will be necessary before further action will be taken. Ramp systems will comply with the following specifications a. All aluminum construction, solid ramp surface with extruded ribbed nonslip surface. b. Standard Ramps must be of 36" and bariatric ramps must be 48" wide c. Handrails 1.5" round aluminum, continuous gripping surface. Contractor shall provide coated/non-coated handrail grip options. d. Minimum level 5x5 landing surface at the door/beginning of ramp, and end of the ramp. Depending on the length of the ramp system, it may be necessary to position a landing midway of the configuration. e. Ramps shall have a 3 year warranty f. Ramps shall land on a hard level surface - Do not land Veteran in the yard - add additional ramping as needed to allow Veteran to reach hard surface (walkway, driveway, patio area), or a vehicle to pull up to ramp for Veteran to load and unload g. Resting platforms at 30' of continuous ramping h. Must have a ratio of incline 4.8 degrees/ 1:12 degree slope - **considerations for grade change must be made and noted on drawings 4.0 Process: The process of providing modular ramps requires several steps to ensure an acceptable level of quality service in addition to providing a quality product. To provide the Veterans with the portable modular ramp systems required, the one stop turnkey process shall be as follows: 4.1. Evaluation Request: The Prosthetic Representative will request a site evaluation to be performed; this request will include the following. a) Date of request b) Name of requesting Prosthetic Representative c) Telephone Number and email of Prosthetic Representative d) Requesting facility name and station number e) Veterans last name and last four of SSN f) Veterans complete address g) Veterans phone number and alternate number if available h) Proposed location of ramp (if known) i) Additional notes or information as necessary The Prosthetic Representative shall place all purchase orders for assessment via fax or encrypted email for the purchase and delivery of a ramp system. Service response time is calculated from the time the Contractor is notified of an individual requirement for services until the Contractor completes all activities at the Veteran's place of residence (delivery and set-up, customer education/ instructions and clean-up). The Contractor shall acknowledge receipt for all assessments, by fax or secured email, within two (2) hours of initial notification by the VHA to ensure prompt service. 4.2. Site Evaluation/Assessment: All communication with beneficiary/caregiver(s) will be in accordance with section 7.1 Notification and Communication with Veteran Beneficiary of the Statement of Work. a) After receiving a request for evaluation, the Contractor shall notify the Veteran beneficiary/caregiver(s) in advance to arrange a mutually agreed upon time for an appointment to conduct a site evaluation for the installation of a modular, or threshold ramp or delivery of a portable ramp system at the designated location. b) A date and time will be set with a four (4) hour window and Contractor shall indicate either a morning or afternoon arrival. The Contractor shall provide a local or toll-free phone number to the beneficiary/caregiver(s) in the event that they have any questions or need to change the agreed upon appointment schedule. c) All customary and usual means of contact will be used to communicate with beneficiary or caregiver(s). If the Contractor is unable to make contact with the beneficiary/caregiver(s) via telephone within a two (2) day period, the Contractor shall ask the VHA Prosthetics Representative for assistance with this task. In the event that the arrival for the scheduled evaluation is delayed (or anticipated to be delayed) beyond the four (4) hour window, the Contractor shall contact the Veteran beneficiary or caregiver(s) and notify them of the delay or need to reschedule. d) Upon arrival for the scheduled appointment, the Contractor/Evaluator shall meet with the Veteran beneficiary or caregiver(s) to determine the most appropriate location for the ramp taking into account space requirements, topography, parking, utilities, property lines, convenience of use and impact on others living at that location. The Evaluator shall also determine if any additional considerations are needed in the design to accommodate Bariatric Veterans. e) Once the best location has been mutually determined, the Evaluator will take the required measurements and sketch the area being evaluated. Any obstacles, landscaping, or other factors that will affect the placement of the ramp will be noted on the evaluation sketch. A digital picture will then be taken of the proposed location from a minimum of three (3) angles to show the entrance and the surrounding area where the proposed ramp will be located. f) The Evaluator shall then explain to the Veteran beneficiary or caregiver(s) the type of product proposed and a suggested configuration based on the initial evaluation to determine if there are any concerns or objections to the proposed location. If there are no concerns stated at this time, the Evaluator shall obtain the veterans signature approval of the proposed configuration. There may be instances where the veteran's property needs to be moved or removed prior to the installation of the ramp system. In these cases, any and all conversations and relating information shall be documented by the contractor and submitted with the assessment. The Contractor shall notify the Prosthetic Representative by telephone upon discovery of safety or environmental hazards that shall prevent the Contractor from installing the ramp system until the hazards have been corrected within 1 business day. VHA shall work with the Veteran to abate the condition preventing the set-up of the ramp system. Upon completion of the evaluation, the contractor shall provide the beneficiary/caregiver(s) their contact information should any questions arise. g) In the event the beneficiary/caregiver and evaluator do not agree on the proposed ramp location (excessive ramp runs, unsafe location etc.), evaluator must notify local Prosthetic Representative for further instructions. The evaluator shall also notify the Prosthetic Representative if it is determined that the beneficiary's residence is not conducive to a modular portable ramp, and warrants a vertical porch lift; contractor shall invoice for the assessment only. h) The Contractor shall complete and submit an assessment of the proposed site of installation within three (3) calendar days starting from the next working day after receipt of request from the ordering VA Prosthetic Staff. i) The assessment shall include: site pictures taken at a minimum of three (3) angles, drawing(s) showing existing conditions and layout of proposed ramp, an itemized quotation of supplies/services for the proposed ramp installation, and the veteran's signature acknowledging approval. j) Pricing for a ramp system on the quote shall be itemized based on a per section price to include shipping and installation fees in accordance with the Schedule of Items. Any custom components that may be required to complete a job that is approved by the VHA Contracting Officer shall be noted on the quote. k) The Prosthetic Representative shall notify contractor in the event a ramp installation has been cancelled after assessment has been completed. 4.3. Purchase Order: a) Once the Contracting Officer or designated VHA Personnel has accepted the contractor's quote, a task order will be faxed or emailed to the Contractor. The Contractor shall acknowledge receipt of the task order which will start the Ramp Installation procedure. Under no circumstances shall the Contractor begin production, shipping or installation without having a valid, signed task order from the Contracting Officer or designated VHA Personnel. A VHA Contracting Officer or VHA designated Personnel is the only person who can authorize a job to begin. b) In the event that circumstances change after placement of order, but prior to installation (a veteran no longer desires a ramp system and the VHA Contracting Officer or designated VHA Personnel cancels the job), the VHA will compensate the Contractor for the costs related to the order processing. Order processing is considered to be; any custom manufacturing of components, shipping and re-stocking of materials. In the event this situation occurs, the Contractor shall submit an invoice itemizing these charges to the Contracting Officer for approval and payment; items will then be placed in government inventory for 1st use priority of the next ramp installation. 4.4. Ramp Installation: a) As outlined in the site evaluation, the Contractor shall schedule with the beneficiary or caregiver(s) a mutually agreed upon date and time to perform the installation of the approved ramping system. This appointment will be set and the installation performed within ten (10) calendar days after receipt of VHA task order. b) The Contractor shall install the ramp at the predetermined location at the beneficiary's home per approved quote and specifications. The Contractor is authorized to install the ramp system only. Requests for additional work to be completed, that do not fall within the scope of the ramp installation and that is not specifically detailed in the approved quote or authorized by the Order, shall not be discussed with the beneficiary or caregiver(s) but referred to the Prosthetic Representative placing the order for consideration. c) The ramp shall be installed in accordance with all Federal, State, Local, and VA authorization design guidelines for ramps, including slope, width, and handrails. Such compliance includes the obtaining of any necessary permits or authorizations required under that jurisdiction's local code. No additional ramp components or parts will be installed without prior authorization from the Prosthetic Representative at the authorizing VHA. d) The Contractor shall be responsible for site preparation and cleanup for ramp installation only as stated on the site evaluation and approved by the VHA. e) Once the ramp system has been completed, the Contractor shall communicate, both written and verbally, with the beneficiary or caregiver(s) the safety and maintenance features of the ramp system. The Contractor shall also ensure the Veteran beneficiary sign a statement acknowledging receipt of education on ramp's safety, proper care of equipment, and a certifications statement that all work was completed to the Veteran beneficiary or caregiver(s) satisfaction. These documents shall be forwarded to the Prosthetic Representative. f) Digital "after install" pictures shall be taken of the ramp system from a minimum of three (3) angles showing the entrance and the complete system and grade area. g) Upon completion, the installation information (including the digital photos, signed statements, education provided, and a copy of the invoice) shall be sent to the Prosthetic Representative placing the order within 3 business days. h) The Contractor shall ensure Installers are certified and trained for the installation and maintenance of their ramping system and conform to the standards and service parameters contained in the statement of work. i) For the protection of patient health information (PHI), the Contractor and any parties or agents under their control or performing work under a task order shall be required to comply with a Business Associate Agreement (BAA) established with the Contractor and the VHA. j) All ramps shall end on concrete, gravel, blacktop, or other solid surface that allows the safe travel for the Veteran. No ramp shall end in dirt or grass without prior written approval of Prosthetic representative and signed consent of the veteran or their caregiver. k) In the event installation is done when there is snow ground cover, the contractor shall return within six (6) months or after snow thaw (whichever is earlier) to determine and make any adjustments necessary for safety concerns. l) When a modular ramp cannot be constructed in a safe, logical and cost effective manner, the Contractor shall notify ordering Prosthetic staff no later than the next business day so not to delay provision of alternate safe ingress/egress from Veteran's home any longer than necessary. The Contractor shall notify the Prosthetic Representative by telephone upon discovery of safety or environmental hazards that shall prevent the Contractor from installing the ramp system until the hazards have been corrected within 1 business day. VHA shall work with the Veteran to abate the condition preventing the set-up of the ramp system. 5.0 Removal and Recovery of Modular Ramp Systems Recovery of modular ramp systems shall be done at the VHA's request. Recovery from the VHA beneficiary residence shall be done within ten (10) calendar days after receipt of the order from the VHA medical center from which the original order came. All ramp runs, landings, platforms, handrail systems, guardrail systems, and support legs/footplates shall be cleaned, inspected, inventoried, and restored to reusable condition by the Contractor within ten (10) calendar days after recovery. 5.1 Inventory The Contractor shall store the recovered equipment and establish a tracking system for inventory of ramp runs, landings, platforms, handrail systems, guardrail systems and support legs/footplates. A tracking system shall be established and track equipment per facility. An inventory shall be taken by the Contractor on the 30th of each month and be submitted to the respective VHA facilities by the 10th calendar day of the following month. Recovered inventory is Government owned property and shall be assigned a category by the Contractor of either "active", "inactive", or "quarantine" status. All recovered product shall be used where applicable with the next assessment within the same facilities catchment area , only as a part of a proposed ramp layout. 5.1.1 An active inventory represents recovered product, restored to contract specifications as listed: " Complete ramp shall include the ramp, double bar handrails, posts, curb edges and support legs/footplates and all hardware. " Complete platform shall include the platform, double bar handrail, posts, curb edges/edge protection and support legs/footplates, hardware and or ground pad. 5.1.2 An inactive inventory item represents recovered product that has been restored to the contract specifications listed above for "active" inventory but is missing a component required for active status. The inventory shall also indicate what components are missing for items that are assigned an inactive status. 5.1.3 A quarantined inventory represents recovered product that cannot be restored to contract specifications. Contractor shall store quarantined components for a minimum of twenty-one (21) calendar days from when reported on an inventory report. Inventory shall be disposed of in accordance with FAR clause 52.245-1. 5.1.4 Contractor Storage Area - The Contractor shall be held responsible for the proper placement and security of all Government-owned modular ramp systems to insure against loss or damage while the ramp systems are located at the Contractor's facility. The Contractor shall store Government-owned ramp systems separate from Contractor-owned equipment and supplies. 5.2 Upon expiration of the contract, Contractor shall return inventory to VHA or designated representative (successor contractor) for disposition or be given permission to dispose of quarantined items by the ordering VHA Facility 6.0 Acceptance of Government Purchase Card: Government Purchase Card is the primary and preferred method of payment. As a result the Contractor shall be able to accept the Government Purchase Card for payment. 7.0 Notification and Communication: The Contractor shall refrain from discussing decisions to repair/replace items of equipment or any services not specifically delineated in this order with Veterans or caregivers. Any concerns raised by the Veteran or caregiver outside the scope of this order regarding services provided shall be addressed to the Prosthetic Representative placing the order for resolution. 7.1 Notification and Communication with Veteran Beneficiary: a) The Contractor shall notify each Veteran and/or caregiver, in advance, to arrange for a mutually agreed upon time for an appointment to assess, evaluate and install the ramp system. The Contractor shall visit a Veteran's place of residence in performance of this contract by appointment only, between the hours of 7:00 a.m. and 7:00 p.m., local time. b) The Contractor shall schedule an appointment at least one (1) business day prior to the appointment. c) Appointments shall be made within four (4) hour time blocks. d) The Contractor shall provide the necessary contact information in order for the Veteran to communicate with the Contractor if any questions arise or changes need to be made with the appointment. e) In the event of delays or anticipated delays, the Contractor shall notify the Veteran or caregiver of the delay or the need to reschedule, the Veteran and the Prosthetic Representative shall be notified at least two (2) hours prior to the appointment. f) If the Contractor is unable to make contact with the Veteran or caregiver within a two (2) day period, or if the Veteran chronically fails to meet appointments, the Contractor shall contact the VHA Prosthetic Representative for assistance. The Contractor shall use usual and customary means in order to make contact. Physical visitation to the Veteran's home is not required to meet this requirement. g) If the Contractor is unable to locate Veteran due to change of address, or for any other reason, the Contractor personnel shall contact the appropriate Prosthetic Representative or designee, within 1 hour, for further instructions. h) Refusal of Service - all Veterans have the right of refusal of service. In the event a Veteran refuses service or orders the equipment to be removed from the home, the Contractor shall comply with the Veteran, only after receiving authorization from VHA. However, the Contractor shall provide a written report, which includes the Veterans signature on the visit form indicating refusal. The Contractor shall report the specifics of the refusal to the Prosthetic Representative, in writing, within one (1) business day. 7.2 Changes/Adjustments: Contractor shall inform beneficiary that a 3 day grace period will be given for any changes or adjustment to the ramping system due to a veteran or caregiver concern. Adjustments will be made at no charge but any additional materials required will be quoted to the requesting facility for approval prior to installation. 8.0 Reports: The Contractor shall submit reports in accordance with the templates provided in Attachment ** to the Contracting Officer, Contracting Officer's Representatives or designated Prosthetic Representative based on the required report. 8.1. Cost Savings Report: Itemized listing of systems sold to VISN/Region showing savings compared to standard pricing. Contractor to be provided on a quarterly basis to the CO. 8.2 Incident Report: All accidents, malfunctions, injuries, or deaths associated with the provision, delivery and use shall be reported telephonically to the Contracting Officer, the facility Prosthetic Representative) and equipment manufacturer, if applicable, within one (1) hour of the event. A written report shall be followed within 48 hours of the incident. 8.3 Inventory Report: Listing of all government recovered equipment shall be submitted monthly to the CO, and applicable COR/Prosthetic Representative by the tenth calendar day of the following month. 8.4 Personnel Report: Listing of all personnel performing services under this contract shall be submitted to the CO, and COR's. This report will be due prior to any services provided. Additionally this report will be due bi-annually. Any individual changes to personnel in between reporting times shall be emailed to the respective COR's , and changes captured on next report. 8.5 Contractor Quality Management Report The Contractor shall maintain a Quality Management Plan demonstrating organizational improvement. This shall entail monitoring of satisfaction, data analysis and collaborative design and operationalization of improvement opportunities. The Contractor shall evaluate the quality of services and document the findings, conclusions, recommendations and corrective actions taken. The Contractor shall provide a written bi-annual report to the CO/COR's covered under this contract for improving organizational performance. First report shall be due six (6) months after contract performance begins. Subject report shall include information on special incidents, Veteran satisfaction, and performance improvement. The Contractor shall collect and aggregate the customer service satisfaction data on service satisfaction from Veteran and their families. Copies of the aggregated data shall be forwarded to each COR by the due date listed below. 9.0 Warranty: Contractor shall provide a warranty on materials and workmanship on all ramping products. Warranty information shall be made available to and acknowledged by the Veteran beneficiary/caregiver(s) and included with or on each invoice submitted for payment. The warranty shall be for a minimum of 36 months. 10.0 Equipment Recalls: Upon notification of equipment recalls, the VHA shall inform Contractor by written notice within five (5) business days. The Contractor shall take the necessary corrective actions and inform each VHA facility in writing of all actions taken within ten (10) calendar days. The Contractor shall in turn notify each VHA facility COR of all equipment recall notices received by them within fifteen (15) calendar days of initial notification. 11.0 Performance and Conduct: A high degree of professionalism and understanding shall be demonstrated when contact with Veteran beneficiary is made by the Contractor's employees. Contractor employees shall be dressed in attire that identifies the Contractor. Contractor's employees are expected to be discreet and tactful and demonstrate concern, compassion, and patience. Some patients have physical disabilities or chronic illnesses that influence their behavior and lifestyle. Any verbal or physical abuse, or unprofessional behavior or conduct toward a patient and/or caregiver shall not be tolerated. The VHA reserves the right to request the removal of any employee from performance of services under this order if his/her behavior and/or level of services provided are not in strict accordance with the requirements of this contract. 11.1 Employee Identification: All Contractor employees shall wear identification badges when providing services at a Veteran's place of residence. Prior to entering a Veteran's place of residence, Contractor employees shall present a photo identification badge to the Veteran/caregiver and verbally identify themselves. The badge shall include the company name, employee's name, and employee's picture. Only Contractor's employees that have been identified in writing to the Contracting Officer shall be allowed to provide services at a Veteran's place of residence. 12.0 Personnel Security:: Contractor must ensure that all personnel who visit a Veteran's place of residence have undergone a background screening with all appropriate adjudications, when required by law or regulation. Contractor must maintain accurate and up to date files on personnel who visit a Veteran's place of residence. The Contractor must ensure that services are not provided, at any time, by personnel who are not properly adjudicated by the company. ? If your firm does not have current or past experience providing services for the Department of Veterans Affairs please list relevant past experience providing assessment, ramp, and installation services 4. What is the highest total number of portable ramps and installation services that your firm can or has assessed, delivered, and installed in one month? 5. What is the highest contract value for assessment, portable ramps, and installation services your firm has been awarded and managed? 6. Do you have the capability to provide service to territories and other than the lower 48 states (i.e. Philippines, Puerto Rico, Guam, Hawaii, and Alaska)? 7. What is the largest area or number of states for which you have/can provide assessments, portable ramps, and installation services concurrently? 8. Do you currently have the necessary resources to provide for the assessment, portable ramps, and installation services that cover a VISN, Region, or Tri-Regional contract? If not would you be able to obtain the necessary capital, human resources, management staff, equipment and supplies within 30 days of contract award? 9. Does your firm have experience/policy providing for assessment, portable ramps, and installation services in rural areas and within intense heat or harsh cold inclement weather conditions? 11. How much of the performance under the assessment, portable ramps and installation services requirement would your firm need to subcontract in order to provide the necessary assessment, portable ramps and installation services requirement? 12. (a) What are the dimensions of each of your component parts that encompass a portable ramp system (widths, lengths of platforms, rails, threshold etc.) (b) What styles of ramps (i.e. perforated, solid, aluminum, steel). 13. In the commercial market place is pricing all inclusive to include assessments, ramp components, and installation? If not, how are they priced separately? 14. Do you manufacturer or distribute one specialty of ramps or numerous different ramps. Please explain and cite examples. 15. If only a manufacturer, will you provide assessment and installation, or how will you perform assessment/installation? 16. If only a distributor, who do you distribute from, and will you provide assessment and installation, or how will you perform assessment/installation? 17. If a distributor, are there agreements/contracts in place with manufacturers? 18. Can you provide removal and recovery services to include the ability to track components at the individual facility level? (a) What is the estimated cost per-square-foot for the storage of the ramps that may require removal and recovery services? 19. If you have an electronic database, what reports are available? Are the reports static or dynamic? 20. Does vendor hold a current FSS contract in SIN 65 II F, Portable Mobility Devices, F-8, Portable Ramp Systems? If so, please provide a copy. (a) Does the vendor's FSS schedule include the installation of the ramps that are ordered as a priced contract line item number? (b) How is installation priced (i.e. hourly rate of by linear foot)? (c) If the vendor's FSS schedule does not include installation, can the vendor provide installation on an open market Contract Line Item Number (CLIN)?
