67--Special Notice-Notice of Intent: Digital Imaging Plates

This is a notice of intent to issue a sole-source procurement action to Carestream Health, 150 Verona Street, Rochester, NY 14608 on a sole source basis under the authority of FAR 6.202-1, Only One Responsible Source and no Other Supplies or Services will Satisfy the Requirement. The proposed procurement is for (20) 14 quote mark x17 quote mark Carestream Industrex Flex GP Digital Imaging Plate/Non-Medical/S0-170 (Part Number 1008564) and for (4) 14 quote mark x51 quote mark Carestream Industrex Flex GP Digital Imaging Plate/Non-Medical/S0-170 (Part Number 1432772) to support ballistic and static testing. This combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice; a written solicitation will not be issued. Interested persons may identify their interest and capability to perform by responding to this notice by the specified closing date. This notice of intent is not a request for quotes; however the Government will consider all quotes received by the closing date specified in this notice. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement. The following provisions apply to this acquisition: 52.212-1, Instructions to Offerors - Commercial. The offeror must include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with its offer. 52.212-4, Contract Terms and Conditions -- Commercial Items. 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items. 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports. 252.212-7001 Contract Terms and Conditions Required Implementing Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items. 52.006-4400 Intent to Solicit Only One Source The Government will award a contract resulting from this notice to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following two (2) factors shall be used to evaluate offers: (1) Technical capability and (2) Price. Award will be made to the lowest price technically acceptable offer. In accordance with FAR 5.207, all responsible sources may submit a quotation which shall be considered by the agency.
