This is a notice of intent to sole source to American Institute For Preventative Medicine for the printing of logos of patient informational books throughout VISN 9. This request has already been fulfilled in other VISN'S by this company and due to standardization of the VISN'S need to have all of the books the same for our Veteran's. STATEMENT OF WORK Veterans Health at Home: Your Lifetime Guide STATEMENT OF WORK PART A – GENERAL INFORMATION A.1 INTRODUCTION –This is a book developed to assist patients in making informed choices about their health. A.2 BACKGROUND –It was reviewed by the V9 Outpatient committee and determined to be of great worth for the patients to use in their homes. The targeted group for this release are the patients in the MOVE program. A.3 SCOPE OF WORK – 1000 manuals will be distributed to each or V9 six facilities to the MOVE Coordinator for patients enrolled in their program. STATEMENT OF WORK PART B – WORK REQUIREMENTS B. There are no training requirments STATEMENT OF WORK PART C – SUPPORTING INFORMATION C.1 Place of Performance –.The books will be distributed to the V9 Lead MOVE Coordinator at each medical center.
