The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. The Great Lakes Acquisition Center is planning to solicit for a vendor that can provide a Virtual Fitness System for training patients at the Tomah VA Medical Center located in Tomah WI. The Great Lakes Acquisition Center/ VA Milwaukee is conducting a market survey to determine the existence of potential small business concerns (e.g., HUBZone, 8(a), SDVOSB, Woman Owned, etc.) with the capability to accomplish this Service. The contractor shall provide the following services for continuation of the streaming & virtual class offerings: Provide 16 channels-- Bar Strong, Bosu, Definitions, eXpress, Fusion, Kettle Power, Kinetics, Office Breaks, Pregnancy Express, Rev, Fusion, Time Savers, TKO, Transitions, Vibe and Zumba; 7+ Workout Plans; Silver & Fit Package to include 12+ Classes; licensing, service & support for one year. User Streaming for up to 75 Users Interested parties shall provide a brief overview of their firm s capabilities and past experience All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments. Each submittal shall also include the following business information: DUNS number Organization Name Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size Socio-economic status (IF SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) Firms shall identify whether or not they are a small business or other than small business in accordance with the size standard for NAICS code 812990.
