UNIFORM PROGRAM The Southeast Louisiana Veterans Health Care System (SLVHCS) is a Complexity Level 4A medical facility and teaching hospital, providing a full range of patient care services complete with state-of-the-art technology, education and research. Comprehensive health care is provided through emergency medicine, primary care, tertiary care, and long-term care in the areas of medicine, surgery, mental health, physical medicine and rehabilitation, neurology, oncology, dentistry, geriatrics, and extended care. Responses are due no later than Wednesday, October 17, 2018 @ 2:00 p.m. CDT. Instruction to Offerors Guidance: This Sources Sought Notice (SSN) is not a Request for Quote (RFQ). No solicitation is being issued now. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the government to an acquisition approach. Any information provided to the Government because of this SSN is voluntary. Responses will not be returned. No entitlement to payment or direct or indirect costs or charges by the Government will arise because of contractor submission of responses or the Government s use of such information. This Request for Information is for planning purposes only and is intended to identify contractors in all categories who can support this requirement. Respondents should include as part of their submission: Indicate whether or not you can support the procurement requirement based on the items identified in the Performance Work Statement (PWS); Any questions that you have, reference this Sources Sought Notice; Company point of contact name, telephone number and email address; Provide DUNS number, CAGE code, and indicate whether or not you are registered in the System for Award Management (SAM) database (Website: www.sam.gov).
