Reupholster Office Chairs

This is a Request for Information (RFI) only This is not a Request for Proposal (RFP) or Request for Quotation (RFQ) In accordance with FAR 15.201, the Indian Health Service, Phoenix Indian Medical Center at 4212 North 16th Street, Phoenix, Arizona 85016 is conducting market survey to identify contractors for the following: Remove existing upholstery, staples, reupholster, install foot gliders and refinish/recover wooden open arm chairs for 22 office guest chairs. Interested contractors who meet the criteria above may submit information to the Acquisition Department. This RFI is open from January 7, 2015 through January 10, 2015 by 9 am. This RFI is solely for information and planning purposes only. Information that is to be submitted by email shall include the following: • Company Valid Data Universal Number (DUNs) • Is your company a Small or Large Business • Information on how you company can successfully perform the requested services. • Name of Contact person with phone number, email address and address.
