Page 1 of 1 SOURCES SOUGHT NOTICE For Adult Day Health Care (ADHC) for VA Northern Indiana Healthcare System This is a Sources Sought Notice. This advertisement is not a solicitation for offers, nor is it a request for quotes. A request for quotes may be issued at a later date. The purpose of this Sources Sought is to conduct market research to determine if responsible sources exist and if there are any businesses capable of providing the requirements. The proposed NAICS Code is 624120, Services for the Elderly and Persons with Disabilities ($11 Million). The Northern Indiana VA Health Care System (NIHCS), in Fort Wayne/Marion, Indiana has a need for Adult Day Healthcare services to all eligible veterans of the VA Northern Indiana Healthcare System. The Contractor shall provide all necessary personnel, equipment, and supplies associated with the full range of ADHC services in performing the requirements of this contract. Contractor's facility shall be located in the Primary Service Area of VA Northern Indiana Healthcare System, in order to service up to 5 veterans per day, five (5) days a week, Monday through Friday. This is a total need and the VA contemplates making multiple awards (If applicable). The government is interested in sources that can provide the required services.
