THIS IS A SMALL BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should not submit a response to this notice. Small business concerns should be capable of furnishing all labor, supervision, and vehicles to support the specifications. The NAICS code is 485991 - Special Needs Transportation DESCRIPTION OF THE REQUIREMENT: PERFORMANCE BASED STATEMENT OF WORK The Contractor shall furnish the necessary personnel (driver) and equipment (vehicle) to provide special needs transportation for wheelchair patients and sedan transportation services to the beneficiaries of the Charlie Norwood Veterans Administration Medical Center (AUGUSTA VAMC) located at the downtown division 950 15th St, Augusta, GA 30904, and the uptown division, Number One Freedom Way, Augusta, GA 30904, and the Community Based Out-Patient Centers (CBOC) located in Athens, GA , Statesboro, GA, and Aiken, South Carolina. Service will be from veteran's residence to Augusta facilities and back, between Augusta Divisions and to include facilities providing care at VA expense. This service shall include bariatric wheelchair patients and high-back wheelchair patients. The contract period is for a base year with four (4) one year option renewals. Actual quantities may vary. The AUGUSTA VAMC reserves the right to increase or decrease the actual requirements. Service is to begin on or about January 14, 2014.
