V--Contract for outside of Milwaukee County wheelchair van patient transportation.

The Clement J. Zablocki VA Hospital, 5000 W National Ave, Milwaukee, WI 53295 is seeking potential sources capable of providing all personnel, equipment, and material necessary to provide for wheelchair van transportation on an as needed basis. Services to be conducted Monday through Sunday, 24 hours a day, including Federal Holidays to and from locations as designated by the Beneficiary Travel Office and the Administrative Officer of the Day. The driver shall assist the patient, as necessary, to enter and exit vehicles, enter and exit domicile; and to enter, locate, and sign-in at appropriate clinic areas upon arrival at destinations. Vehicles transporting patients shall not carry non-VA contract patients or private pay patients with VA patients. Most trips will be with a starting or ending point in the surrounding counties of Milwaukee including but not limited to Waukesha, Racine, Ozaukee, Walworth, and Washington counties and start or end at the Milwaukee VA Medical Center. Trips may also have a starting and ending point within the above stated counties for Non-VA care appointments. Contractor will also provide transportation services for patients with appointments at other health care facilities in the Madison, Tomah, Iron Mountain, or Chicago areas. One way trips will not exceed 5 hours from the Milwaukee VAMC or patient's place of residence. Contractor would be required to provide services in accordance with all Federal, regional, state, and local regulations. The applicable NAICS is 485991, Special Needs Transportation, with an applicable size standard of $15.0 million dollars. No formal solicitation document exists at this time.
