VA Mid-Atlantic Health Care Network (VISN 6)Television Replacement Initiative Statement of Work (SOW) In 2009, VISN 6 intends to enter a contract with a vendor to purchase and install two types of televisions: LCD Televisions and Articulating Arm Personalized Televisions in VAMC Asheville NC, Beckley WV, Durham NC, Fayetteville NC, Hampton NC, Richmond VA, Salem VA, Salisbury NC and their associated outlying patient clinics. VISN 6 Television Initiative estimates purchasing approximately 1100 1300 23 to 26 LCD Televisions, 150 200 32 to 42 LCD Televisions and 150 to 200 15 Articulating Arm Personalized Televisions. Majority of televisions are a one for one replacement. But in patient shared rooms, additional televisions will be purchased and installed in these areas. Examples of patient shared rooms: two patients in one room sharing one television, the procurement will add an additional television giving two patients in one room two televisions. Four patients sharing two televisions in one room, the procurement adds two additional televisions (4 patients, 4 TVs in one room). Vendor is responsible for the complete installation (turn-key) of the televisions. Turn-key installation includes all cabling for the distribution system. Cabling will be connected to existing distribution system. Vendor is responsible for installing brackets (wall, ceiling and/or cart), providing amplifiers in areas with weak signal strength, providing additional electrical power, receptacle / outlet, coaxial jack (solid cooper center), interfacing the TV with the pillow speaker and raceway in areas converting a shared room with a television to one to one TV solution. Personalized televisions (articulating arm) may also require reinforcing walls to support articulating arm and television unit. Purchase of the televisions must include a trade-in allowance (credit price) and removal for any television in the medical center. Attached spread sheet list the trade-in information (S/N or EE #, mfg/model, age). Vendor is responsible for coordinating delivery, installation and removal of the televisions from the medical centers. This may include positioning of a trailer if necessary to accomplish this task. Televisions (new or old) may not be staged in the warehouse for more then 48 hours. All Televisions must be: UL Hospital Grade Listed with Hospital grade AC Cord Energy Star Label Articulating Arm / Personalized Television (Low Voltage w. power supply) 15LCD Display Request vendor capable of fulfilling this requirement to provide past performance, situational experiences and overview how you would complete this requirement.
