This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Requirements (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation will not be issued. The purpose of this contract is to build on the work from Contract # 75D30121C12121 for the school-based active surveillance among school children. As such, CDC is proceeding with a sole source award to The National Association of School Nurses (NASN) for Phase 3 of the School-Based Active Surveillance (SBAS).
