SCOPE OF WORK ID #: GL2014-020 To Provide Beverage Service Adams and Associates, Inc., a Prime Contractor for the U.S. Department of Labor for the operation of the Glenmont Job Corps Center - a student residential training facility located at 822 River Road Glenmont, NY 12077, is interested in negotiating a Blanket Purchase Agreement (BPA) for the items listed in the attached Statement of Work (SOW). Offerors should note that this is an agreement with Adams and Associated, the Glenmont Job Corps Center and is not an agreement with the U.S. Department of Labor. Site Visit Date: 04/03/2014 at 10:30AM All quotes must be submitted in writing to the Purchasing Department by Monday, April 10, 2014 by 3:00 PM Quotes received after the deadline and/or without all requested documentation will not be considered. Vendor is to return bid with company name and address at the top of each page; vendor questionnaire and W9; all licenses for doing business in the State of New York, Certificate of Liability insurance and list of references if no past experience with Center. The period of performance is May 1, 2014 to December 31, 2014. Bidders should be aware of bid requirements for federally funded projects and act accordingly. Bidders are placed on notice that this is a competitive bid process and cost is an important fact. SCOPE OF WORK ID #: GL2014-020 Vendor is to Provide Beverage Service Freight, fuel surcharge or handling charge, if any, must be noted on bid or charges will not be paid if invoiced. Vendor is to provide beverage service as follows: • Provide post mix fountain soda dispenser with ice at no charge. • Provide post mix fountain juice dispenser at no charge. • Provide post mix coffee dispenser at no charge. • Eight 20lb CO2 tanks are to be provided to operate the complete system. Approximate Monthly Delivery: • 60 gallons of post mix Soda to include: Diet, Ginger Ale, Lemonade and Iced Tea. • 35 gallons of Juice to include: Orange, Cranberry and Fruit Punch. • 10 cases of Coffee to include: Regular, Hot Chocolate and French Vanilla. Vendor shall hold all licenses necessary to do business in the state of New York and/or state business is located in and provides copies of all licenses with bid package. Vendor must supply manufacturer name and stock number for all items bid on. Items reference a manufacturer and stock number on bid sheet for reference purpose to insure correct item is bid. Vendor may substitute identical items under a different manufacturer or brand name but must note manufacturer and stock number on bid sheet. Vendor is to note on bid sheet if item is a green or recycled item. Vendor is to provide a billing invoice that includes BPA number and shows individual lines for each item ordered. Glenmont Job Corps Center does not accept backorders all orders will be considered complete upon receipt. Vendor must make Purchasing Agent aware of all shortages, via email

The vendor shall obtain and maintain insurance as follows if vendor's employee(s) or agent(s) will be on Center at time: • Workers Compensation • Comprehensive General Liability with Glenmont Job Corps Center listed as additional insured. ($1,000,000 per occurrence). • Comprehensive Automobile Liability ($1,000,000/combined single limit) • Fire Legal Liability ($500,000 per occurrence) • The proposed shall cover any expenses not covered by said insurance including all deductibles.
