This is a combined synopsis/solicitation for commercial service. This announcement constitutes the only solicitation. The California National Guard has an immediate requirement for Medical Surveillance Physical Exams to satisfy OSHA and Cal OSHA ha zardous materials requirements at Camp Roberts, CA for troop mobilization. The California National Guard intends to award a Blanket Purchase Agreement (BPA) with one base year and 4 option years for these services. ALL VENDORS MUST BE READY TO BEGIN EXAM S NO LATER THAN OCTOBER 13, 2004. GOVT IS UNABLE TO PROVIDE BUILDING SPACE TO THE CONTRACTOR. CONTRACTOR MUST PERFORM EXAMS AT A STATIONARY (FIXED) LOCATION THAT IS LOCATED WITHIN 50 MILES OF CAMP ROBERTS, CA. REQUIRED COMPONENTS OF THE MEDICAL SURVEILL ANCE EXAM: *Comprehensive Occupational Health History  needs to have specific emphasis on prior potential exposures to cholinesterase inhibiting substances (e.g. organophosphates); and chemicals associated with cardiovascular, pulmonary, neurological, or psychiatric disease. * Medical History  Review of Systems focusing on the skin, eyes, pulmonary, cardiovascular, and neurological systems. * General Physical Exam  Physician administers a general physical examination with emphasis on the diagnosis of p ossible disqualifying cardiovascular or pulmonary disease and to detect any significant abnormalities in visual acuity or hearing, the skin, or central nervous system. * Resting EKG Interpretation - Members will have a recent resting 12-lead EKG with them in the medical packets that they bring with them to the physical exam. * Stress EKG (Treadmill)  at the discretion of the examining physician, an individual may obtain a stress EKG if the individual is to perform strenuous activities using protective cl othing and equipment. * Audiometric Evaluation  to determine individuals auditory acuity. Test modality must comply with OSHA / Cal OSHA standards. * Occupational Vision  Determination of near and distant visual acuity and pupil reactivity. For me mbers over the age of 35  intraocular pressure measurement is required. * Increased intraocular pressure will require an ophthalmologists evaluation and clearance to work in areas where potential exposure to organophosphates exists. * All individuals must have corrected near and distant visual acuity of 20/40 or better in at least one eye. LABORATORY REQUIREMENTS: * Complete blood count with differential *Routine chemistry panel *Thyroid panel *Blood lead *G6PD *RBC cholinesterase *Routine urinalysis *Urine for heavy metals *Other tests to be added or subtracted at the discretion of the examining physician in accordance with applicable OSHA and Cal OSHA regulations. IMMUNIZATIONS REQUIREMENTS: Updates may be needed for certain routine immunizations. This synopsis is 100% set aside for small business. Simplified Acquisition Procedures will be utilized.
